SPECIALIZED FOSTER CARE PROGRAMS
Parent Model Plus Program (PMP)
The Parent Model Plus program is the base rate program designed to serve Foster Children/Youth being admitted to foster care who have social, emotional, educational, medical or developmental needs associated with being at risk and in need of child protection services. Each and every Foster Child/Youth will receive the level of support required to meet their needs. Based on the assessments completed by the service team, the individualized service plans will be designed and implemented in a variety of ways including access to a specialized and customized support staff team, specialized youth services, medical supplies/interventions, specialized camp programs, respite care, transportation services, and other customized, clinical services.
Mixed Modality Program (MM)
The Mixed Modality Program is designed to care for Foster Children/Youth with multiple exceptionalities. In some cases Foster Children/Youth admitted into the MM program are those who have placement needs that may be suited for group home settings. Foster Children/Youth who are placed in the MM program may have one or more of the following conditions FASD; medical needs; PDD, Autism, ODD and ADHD. Based on the assessments completed by the service team, the service plans will be customized to meet the therapeutic needs of the Foster Child/Youth and will be implemented in a variety of ways. In addition to access to a specialized and customized support staff team, access to specialized youth services, medical supplies/interventions, specialized camp programs, respite care, transportation services, other treatment/clinical services are available.
Mixed Modality Plus Program (MMP)
The Mixed Modality Plus Program is designed to provide an alternative to staff-operated- residential-group care for Foster Children/Youth. Foster Children/Youth placed in the MMP Program may have intense levels of need and challenges in some or all areas of their development. This program is designed to care for Foster Children/Youth with complex-care needs who may be diagnosed with one or more of the following; FASD, PDD/Autism, ODD/Conduct Disorder, and other high risk conditions including drug /alcohol addictions, and mental health disorders. Foster Children/Youth will be cared for within an experienced family home environment while receiving extensive treatment services. Based on the assessments completed by the service team, the service plan will be designed to include the following: specialized and customized support staff team, access to specialized youth services, counselling services, specialized camp programs, respite care, transportation services, other clinical treatment services as required.
RECENTLY DEVELOPED TREATMENT PROGRAM
Treatment Program (TP)
Bridgeway Family Homes (BFH) is pleased to launch our Bridgeway Treatment Program (TP) to serve children/ child/youth with complex care needs. These are children/youth who would otherwise be considered for placement in a residential group home setting. We will provide experienced and trained therapeutic Foster Parents supported by a wide range of clinical, educational and recreational supports. Supporting the children/youth will be a comprehensive group of professionals including: a Treatment Foster Homes Manager, a Treatment Foster Home Supervisor providing 24/7 support; Treatment Foster Home Clinical Consultant, Clinical Assessors, and Child/Youth Support Staff- Known as ARTS Specialized Services (Assess, Resource, Treat, Support); School Success Teams (Certified Teachers, S.E.R.T. Special Educational Resource Teachers); Addiction Counsellors; Access to Psychologists, Registered Nurses, Art Therapists and Psychiatrists as required.
